How to Register with the
Practice
To
register with the practice you need to come to either Twyford or Colden
Common during opening hours and speak to the Receptionist. She will
provide you with a registration form and any other details you may
require.
As a
patient, we are obliged to express a preference to receive services from a
particular performer or class of performer either generally or in relation
to any particular condition and record in writing any such preference
expressed by or on behalf of the patient.
We shall
endeavour to comply with any reasonable preference expressed as stated
above, but need not do so if the preferred performer

has reasonable grounds
for refusing to provide services to the patient or

does not routinely
perform the services in question within the Practice.
A patient
can be removed from the Practice list, with immediate effect, if they
commit an act of violence against any member of the Practice staff, or
behave in such a way that any such person fears for their safety and the
incident reported to the police. |
