New Patient Registration

From March 2020, new patients will need to register online, or receive an emailed copy of the Registration document. You will then be reminded to bring in ID when appropriate. This is to avoid any non-essential visits to the surgery for the time being, to keep the staff and our patients safe. 

Download our registration document Adult Registration Form, then print, complete and scan to surgery email address ease also scan your driving licence and passport, or similar photo ID.

If you require your repeat prescriptions, please sign up to either Patient Access or the NHS App to order your repeat prescriptions online.

Please also complete the Pharmacy Nomination/EPS (electronic prescription service) form EPS Nomination. For more information about EPS, see here.

Once you have registered with the surgery you can sign up for Patient Access or the NHS app, either by downloading the app on your phone or on the website:

You will then need to request a registration letter with the linkage key, to allow you to link the account you've created to our surgery. Please email with your request.
